Terms of submission

1. Introduction

These Cost Model Data Request Terms of Submission (“Terms of Submission”) outline the terms and conditions under which individual(s) on behalf of a care provider organisation (“you”) may submit cost data through our platform (“CarePulse”) which is owned and operated by Evalucom Consulting Ltd.

These Terms of Submission apply in addition to the CarePulse Terms of Use.

By using CarePulse to submit your cost data, you confirm that you accept these Terms of Submission and that you agree to comply with them. If you do not agree to these Terms of Submission, you must not use CarePulse to submit cost data.

We recommend that you save a copy of these Terms of Submission for future reference.

In these Terms of Submission, Evalucom Consulting Ltd are referred to as “Evalucom” or “we” or “us” or “our”.

2. Cost data accuracy

2.1 All cost data and any other information that you submit must be accurate and complete to the best of your knowledge.

2.2 You must read questions carefully before answering.

2.3 You must answer all questions in the units requested and for the time period requested.

2.4 Further clarification of your responses may be required, and we may contact you via email or the telephone number you provide upon registration. You agree to promptly provide any further clarification requested.

3. Data confidentiality

3.1 By submitting cost data on CarePulse, you permit us to share such cost data in aggregated and anonymised form with commissioning organisations (including its staff) to inform price setting. We will not share information identifying your staff and/or your organisation unless we are required to do so in accordance with the CarePulse Terms of Use.

3.2 We will treat the cost data that you submit as confidential and commercially sensitive.

3.3 Multiple users from the same care provider organisation will be able to access the same cost model questionnaire to insert cost data and view answers that have been entered. Access requests to join CarePulse in order to access the questionnaire will be verified by Evalucom before access is granted. We aim to verify all requests within 2 working hours.

4. Data retention

4.1 Personal data that we collect as part of your account registration will be kept according to the retention period set out in our privacy policy.
